You do not need to sign up for any specific plan, and there are no
costs to you, in order to become a patient of House Call Docs, CS

Simply follow these steps:

1. Call 719-632-8787 to discuss any questions you might have and/or to schedule a
   time for the doctor to see you in your home.
   
   
2. Have all health insurance cards readily available for every visit.
   
   
3. Have available the names, addresses, and phone numbers of other physicians
   currently treating you.
   
   
4. Have all current medicines available to show the doctor.  This includes non-prescription medications as well.
   
   
5. Have the following list ready at each visit:

• any prescriptions that need refill authorization;
  
  
• your pharmacy phone number; and
  
  
• a list of questions or concerns that you want to share with the doctor.
